Change of Address
If you are moving house, please let us have your new address, home telephone number and mobile. If you are moving within our practice area please update us in writing, or by sending us an email to wlmc.patientenquiries@nhs.net with proof of address such as a utility bill. Once we have been informed and if your new address is still within our practice boundary, your details will be updated on our system.
Change of Address Outside of Practice Boundary
In most cases, we are unable to keep patients registered once you have moved outside of our practice boundary.
