Support for Patients with Learning Disabilities
Why this matters:
People with a learning disability often face health inequalities. We follow NHS guidance to make care accessible and fair.
What we offer:
- Help to join the Learning Disability Register (no special test needed).
- Annual Health Checks for patients aged 14 and over on the register.
- Health Action Plans after checks to support ongoing care.
- Reasonable adjustments for appointments (longer time, quiet space, home visits if needed).
How to request:
If you or someone you care for has a learning disability, please tell us so we can add you to the register and arrange extra support.
